Incident Reporting (Policy OG-6)

Policy:

This policy outlines the formal process for incidents involving Clarkson College students, visitors, or volunteers (“Participants”). An incident is an unexpected event related to an accident, medical situation, crime, etc., that results in financial loss, property damage, or injury. All incidents not consistent with the routine and expected operation of Clarkson College should be reported. Employee incidents should be reported following the HR-43 Incident Reporting policy to Human Resources. 

  • A student encompasses any person enrolled at Clarkson College. However, a student participating in the work-study program who has an incident that needs reporting while performing work-study duties will use the HR-43 Incident Reporting policy.  
  • A volunteer is any person who donates their time and service without compensation from Clarkson College. 
  • A visitor is any person visiting Clarkson College who is not an employee or student at the College.  
  • For employees who are also enrolled as students, the policy should be followed based on the role that they are operating in at the time of the incident.  

Procedure:

On-Campus Incident

1. In the event that participants are involved in an incident on campus, the incident should be reported to the Designated Personnel (Facilities, Program Director, Clinical Coordinator, Faculty Member, Staff Member, etc.) to complete an Incident Report Form. The following link may be used to access the Incident Report Form. 

2. Clarkson College employees are not to attempt to move an injured person, as this may cause additional harm. They should wait for appropriate medical assistance. 

  • At Clarkson College, shower and eye wash stations are located in the science labs (212 & LL009)

3. Clarkson College employees are not permitted to transport sick or injured participants. The participant may allow a friend, family member, or emergency personnel to transport them to a healthcare facility or other location. 

4. The completed form will be submitted to the Designated Personnel and a copy sent to Clarkson College Facilities at facilities@clarksoncollege.edu.  

5. If medical evaluation is needed, the Designated Personnel will arrange transportation for the participants to the nearest Emergency Services.  

6.If an incident occurs on campus after business hours, an Incident Report Form must be completed within 24 hours. 

Clinical Site Incident

If a student or faculty member is involved in an incident resulting in injury or potential injury in the clinical setting, the incident must be properly documented according to the clinical agency’s policy. 

1. In the event a student or faculty member is involved in an incident on the agency's premises, the agency will provide medical and dental care and/or arrange transportation to receive medical and dental care in accordance with the affiliation agreement. The cost of such care is the responsibility of the injured individual (student or faculty member). The agency will notify the program clinical liaison of the incident. 

2. If the incident is for a bloodborne pathogen exposure/needlestick/injury that requires immediate treatment, follow the appropriate exposure procedures.  

  • Perform immediate first aid. Scrub any wound or affected area with soap and water. Allow any blood to flow freely from the wound. If the exposure is to the eyes, flush with water or normal saline for several minutes.  
  • Notify and report the incident 
    • Report to the Preceptor/Clinical Coordinator/Designee immediately. Also, notify the Program Director or Designee for your program as quickly as possible 
    •  If exposure to blood or body fluid occurs at Clarkson College or Nebraska Medicine or at a facility in the metro area, report the incident promptly to: The Nebraska Medicine OUCH PAGER: 402-888-OUCH (6824). The student’s initial testing and treatment will be covered by Nebraska Medicine when using their services for bloodborne pathogen exposure/needlestick/injury. 
    • If any exposure occurs at a facility outside of the metro area, the facility protocol will be followed using their Employee Health Office OR the nearest Emergency Services. The student/faculty will be responsible for any costs not covered by the agency. 
    •  If exposure to a communicable disease occurs, the student or faculty member will be referred to their primary care provider (PCP) for follow-up. 
    • Note: If exposure is considered high risk and prophylactic treatment is recommended, treatment should begin within 2 hours of exposure. 

3. Complete Incident Report. The student/Clinical Faculty, per the clinical agency policy, should complete an Incident Report Form. Know the following: what, where, how, time, and source.  

4. Follow-up. The Program Director will follow up to ensure receipt of a copy of the agency's incident report. 

5. A copy of the report will be shared digitally with Facilities and additional parties as appropriate.
